Job summary

The successful applicant will work as part of the Community Palliative Care Team and undertake a broad range of nursing support duties relating to the care of patients with end of life care needs in the place where they live, usually their own homes.

They will work under the direction of the Palliative Care Team providing care as required to patients with palliative and end of life needs and will work within the clearly defined boundaries of their competencies.

Main duties of the job

Providing care for named patients with end of life care needs usually in their own homes in accordance with the patient's care plan as defined by the Palliative Care Team with minimum direct supervision.

The Healthcare Assistant will work effectively and flexibly under direct and indirect supervision to meet the patient and family's individual needs.
