Job summary
An opportunity has arisen within our expanding Respiratory and Sleep Sciences Department for a Respiratory Failure Practitioner. The Department of Respiratory and Sleep Sciences is a UKAS accredited Physiology Department offering a comprehensive respiratory and sleep diagnostic and therapeutic service. Due to expansion of service delivery an opportunity to join the team has become available.

Main duties of the job
The Respiratory Failure Practitioner will act as a clinical expert and professional resource to the multi-professional team, patients and carers. The post holder will work as a competent independent practitioner within the Home non-invasive ventilation (NIV) and home oxygen assessment and review services (HOS-AR). They will provide hospital and community based care and support for patients requiring NIV and/or supplemental oxygen in the home. They will undertake independent assessment and management of patients with respiratory failure, where appropriate making clinical decisions regarding patient management and clinical outcomes. They will work closely with the other members of the respiratory failure teams including oxygen, COPD and NIV teams.

Job responsibilities
1. To work autonomously after competency assessment in managing a caseload of patients within the speciality whilst working as part of the multidisciplinary team, delivering individualised and personalised patient care. Appropriate departmental training will be provided.
2. Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed diagnostic procedures/clinical investigations.
3. To undertake a specialist practitioner led NIV and/or oxygen clinic providing assessment for and initiation of therapy and on-going management of patients at home.
4. Support and enable patients and carers to make informed decisions relating to their treatment and management.
5. Provide high quality clinical care and assist in the development of the NIV and oxygen services, through the use of evidence based practice and clinical standards.
6. Participate in the promotion of patient centred care and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and after care (especially after death).
7. Ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
8. Demonstrates the ability to use flexible and innovative approaches to practice, which are consistent with Trust values and appropriate to the patients needs.
9. Participation in the development of clinical guidelines, protocols and policies for goodpractice
10. Use skills in team building to enhance delivery of care.
11. Exercises professional accountability and responsibility in changing environments across professional boundaries.
12. To ensure adequate clinical and managerial records are maintained.
13. To utilise allocated resources effectively and to raise areas of concern to appropriate personnel
14. Provide information, support and counselling to patients and their families.
15. Act as an educational resource for healthcare workers both in the hospital and community participating in formal training as required.
