Job summary

An exciting opportunity has arisen within the renal team for a Band 7 Renal Diabetes Specialist Nurse. The role aims to provide more focused support for renal patients to help assess, support, prevent and treat the onset or long-term diagnosis of diabetes. The role will have a specific focus on supporting renal transplant patients by identifying those transplant patients that are at risk of developing post-transplant diabetes which if managed early can be reversible.

The post holder will be expected to work with the wider multi-disciplinary team and support dialysis outpatients once the role has been established.

The post will require an enthusiast and self- motivated individual and the renal service will provide all essential training and support to fulfil the role.

Main duties of the job

The successful candidate will share our commitment to patient centred care. They will be highly experienced, with advanced skills in managing complex diabetes or renal patients in a variety of settings.

They will be able to make autonomous, evidence-based decisions and lead by example to educate and support colleagues in primary and secondary care.

The post holder will work to proactively manage, coordinate, and support an identified caseload of patients with diabetes who also have renal impairment. This will include patient and carer education, annual health checks and oversight of insulin management

The candidate will be an accessible and responsive team member with clinical responsibility for a defined patient group. able to make informed decisions within their own area and scope of professional practice to ensure agreed standards, objectives and targets are met, with experience in either diabetes care or in renal care.

Training and support will be provided from both Diabetes and Renal Transplant Medicine teams to develop in this role.

A non-medical prescribing qualification is desirable for this post but not essential. If the candidate doesn't already have a non-medical prescribing qualification the candidate must be willing to work towards gaining this qualification in the near future.
