If you are an enthusiastic, motivated EEA Qualified Pharmacist who would like the opportunity to develop their clinical skills and join our team of Clinical Pharmacists we would love to hear from you.

Successful candidates will join the team as an AfC band 5 transitional pharmacist & will receive a Structured Competency Based Training & Development Programme to support you through your transition period (3 to 6 months) & be salary matched to AFC pay band 5. There will then be a guaranteed opportunity to remain in the department as a permanent band 6 as part of our PROPEL Programme if you successfully complete the required training competencies within 6 months of joining.

Upon joining, candidates will gain a wide range of experience in various clinical specialties Medicines Information, Aseptic & patient services alongside completing a funded diploma in clinical pharmacy .

o Providing pharmaceutical care to patients from admission to discharge

o Participating in the multi-disciplinary team at ward level.

o Day to day working with the Medicines Optimisation Technicianso Providing prescribing advice and support to medical staff.

Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.

Main duties of the job

1. EEA Qualified Healthcare Professionals as UK registered pharmacists to join the pharmacy team as an AfC Band 5 transition pharmacist to undertake:

Structured Competency Based Training Programme for 3-6 months

Participate in a locally designed education & training programme & other activities to develop knowledge & skills in line with the professional standards set by the GPhC

Aware of & work within University Hospitals Plymouth procedures, policies accepted standards of practice, relevant legislation & regulatory

Undertake mandatory training deemed necessary to the role

Provide delivery of patient care & pharmaceutical services under supervision

2. Following completion of the 6-month Competency Based programme to join Band 6 to Band 7PROPELstructured rotational development programme to undertake the following duties:

Participate in defined & structured core or specialised rotations across a variety of settings within pharmacy & clinical specialities.

Undertake fully funded further study e.g. Diploma/Certificate in Clinical Pharmacy Practice & Non-Medical Prescribing.

Participate in the delivery of a high-quality clinical pharmacy service ensuring the safe, effective, & economical use of medicines

Provide expert advice on medications to staff across the organisation.

Provide practice supervision of pharmacy staff & other healthcare professionals

Undertake audits & engage with the Department's Quality Improvement projects.

Participate in the on-call & out of hours service

1. Participate in the PROPEL programme (UHP rotational development programme), undertaking defined and structured Band 6 core or Band 7 specialist rotations across a variety of different clinical specialities, as defined above.

2. To participate in all activities of the pharmacy dispensary as required e.g. clinical screening and final accuracy checking of prescriptions.

3. To participate in ward based clinical pharmacy services to support the safe prescribing and administrations of medicines. This may include activities drug history taking and medicines reconciliation, medicine supply, patient medication consultations, medicines optimisation services, discharge medication service (DMS) and any other activities associated with medicines management.

4. Perform clinical check of prescription charts for individual patients and ensure safety promoting medicines optimisation communicating any problems and negotiating solutions with doctors and/or ward staff, in the ward or dispensary.

5. To participate in the production and supply of aseptically dispensed items, whilst working in the aseptic unit.

6. To provide medication related advice to patients, focussing on medicines stopped/started or changed in hospital and side effects profiles enabling them to obtain the maximum benefit from their medicines.

7. Ensure all relevant documentation is completed and document any actions appropriately in the patients notes and / or electronic patient record.

8. To facilitate timely medication availability through participation in, and further development of, the existing patients-own drugs and onestop dispensing schemes.

9. Provide professional responses to medicine information queries from within a clinical service line or designated care area.
