MS Infusion Nurse

Job summary

o Monitor, plan, implement and evaluate expert nursing care and provide support to multiple sclerosis patients and their carers and to the wider CNS Team. To have continuing responsibility for the coordination of individual treatment programmes throughout the illness trajectory and work to develop communication across divisional and organisational boundaries under the direction of Senior Nurse

o To liaise effectively with all members of the multidisciplinary team within the acute and primary care setting

o Act as a role model for excellence in the field of multiple sclerosis

Main duties of the job

o A source of knowledge and information for people with multiple sclerosis

o Provide advice to patients and professionals on multiple sclerosis

o Monitors a caseload of patients with complex needs who require regular Infusions or the administration of sub cutaneous Injections and to administer the MS Medications safely and effectively and in a timely manner.

o Works autonomously and as part of the team

o Provides a link in the care of people with multiple sclerosis between hospital and community services

Contributes to service development, audit and research projects

Job responsibilities

To assist the senior medical and nursing team in managing people with multiple sclerosis within agreed protocols and treatment plans

To administer Disease Modifying Therapies by Infusion or by Subcutaneous Injection to the patients who require regular treatment and Safety Monitoring.

To support a dynamic caseload and monitor response to medication plans including disease modifying therapies

Monitor telephone helplines providing knowledge and advice to people with multiple sclerosis and other healthcare professionals

To support Consultant and Senior MS Nurse led follow up clinics for patients with complex needs To prioritise workload and manage referrals effectively. Review patients and help to support the senior MS Nurses initiate changes in management of care including discharge arrangements

Support protocols on care management for people with multiple sclerosis

To provide advice and support to patients, carers and other healthcare professionals

To assess patients in the clinical area, in relevance to medication profile, MS outcomes and treatment plans

Assess new patients in the clinical area identifying their psychological, physiological, emotional, spiritual and cultural needs in relation to their condition, assist in providing a pathway of nursing care for the patient

Act as a patient advocate, when relevant, enabling the patient to make informed choices over treatment options

Provide continuity of care throughout the patient journey across organisational boundaries

To provide support and empathy for distressed patients and carers, identifying those at risk and referring to appropriate agencies

Maintain accurate records and ensure all relevant information is documented in the medical and nursing notes and on IMED the MS Database.

COMMUNICATIONS & WORKING RELATIONSHIPS Provides up to date and appropriate verbal and written information for patients and carers.

Communicate highly sensitive and emotive condition related information to patients, relatives and carers.

Act as a role model of excellence for pre/post graduate nurses, junior doctors, medical students and other health care professionals
