Job summary

The successful candidate will work as part of multidisciplinary teams at ward and department level, to demonstrate expertise and leadership in the delivery of high-level care to patients with dementia and work collaboratively ensuring the delivery of high quality and safe patient centred care. Whilst leading the ongoing development of multi-disciplinary trust wide dementia services, through leadership of the Dementia Steering Group and managing the Trust Dementia Improvement plan.

The post holder will also lead on the ongoing development of trust wide frailty services through engagement within Frailty steering group alongside other key Frailty and Healthcare for the Elderly team members.

The successful candidate will also lead on the education, training and staff development towards dementia and older people knowledge and skills, working with the Learning & Development team and Service Line Clinical Educators, to ensure that staff acquire and maintain professional standards and competencies.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

To be the lead specialist nurse/practitioner for dementia, frailty and falls pathways in the acute hospital, responsible for leadership towards the developments and improvements to services and care pathways, promoting person-centred care for patients with dementia, frailty and falls in hospital.

Working as an autonomous practitioner, provide specialist clinical assessments, multifactorial interventions, recommendations and advice for the care and management of people with dementia, frailty and falls in hospital

Work as part of multidisciplinary teams at ward and department level, to demonstrate expertise and leadership in the delivery of high-level care to patients with dementia and work collaboratively ensuring the delivery of high quality and safe patient centred care.

CLINICAL

Work with clinical autonomy, deliver and prescribe individualised patient care for people living with dementia, and(or) frailty, across the Trust, involving other members of the multidisciplinary team and other agencies, to ensure robust specialist input and support.

Support clinical teams across the Trust, to provide clinical advice on the assessment and care planning to meet the needs of people with frailty and dementia in hospital this will include specialist advise on aspects of care such as falls risks, behaviour management, nutrition & hydration needs, emotional & psychological care

Work with the Acute Frailty Team, Front Door Frailty liaison team and promoting the speciality Trust wide

Work with HCE Frailty Short Stay ward to promote frailty and speciality assessment for elderly frail patients

Provide specialist Healthcare of the Elderly input to the Frailty Trauma Liaison team and undertaking the specialist MDT ward rounds to provide specialist advice and support to the nursing teams.

Provide advice and clinical education for the care plan/management plan for people with BPSD relating to their dementia in hospital, working with Mental Health Liaison team. Offering advice on the care plan and management of patient- specific needs and behaviours, communicating changes with other health care professionals in the multi-disciplinary team with ongoing responsibility for patient care as they occur.

Provide specialist advice relating to use of Mental Capacity Act for people with dementia including advising on use of Restraining care plans, Enhanced supervision and DOLS applications.

Engage and interact with carers and family, encouraging them to be actively involved in patients care planning as outlined in ward / unit philosophy.

Work with the Mental Health Liaison Team to ensure timely assessment and appropriate care planning for patients with those patients with Dementia who have complex needs or mental health related symptoms.

Work with clinical teams and other specialist nurses to raise awareness and competence in use of Mental Capacity Act in decision-making for people with Dementia - this includes Mental Capacity Assessments, Best Interest decisions, reasonable adjustments to care plans and use of Deprivation of Liberty Safeguards

Provide clinical advice on care for people with dementia for specific clinical decision-making, case conferences and/or discharge planning meetings; also to complaints, incidents or safeguarding alerts relating to people with dementia.

Work with the Discharge Case managers and Integrated Hospital Discharge teams, assist in the assessment of onward care needs for people with Dementia particularly advising on discharge plans for people with dementia needing End of Life care or support with BPSD.
