Senior End of Life Emergency Department Practitioner

Job summary

As continuation of highly successful pilot, the Trust seeks to consolidate and expand the existing End of Life ED team offer. The post offers an excellent opportunity to make a difference at a critical time of life for those in ED (or within pathways to the department).

The post holder will provide expert advice, and support patient care/carers in an area which requires specific clinical knowledge, skills and experience; skills and knowledge will be drawn from the post holders existing professional domain. The post holder will work alongside other clinical colleagues in oncology, palliative care, frailty teams and other wards across the hospital in ensuring best outcomes for those at end of life. As part of plans to extend the service, the post holder will play a key role in developing, implementing and operationalising remote professional advice (pre-conveyance) for those at end of life, or at risk of not surviving admission. In certain circumstances, the post holder will be required to work peripatetically, reaching into community settings, including the regular support of community hospital bed bases.

Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.

Main duties of the job

  • Provide enhanced and streamlined EoL care with ED
  • Consider educational needs across the department/community, imparting contemporary evidenced based knowledge and skills in order to facilitate better care
  • Intervene, support, and facilitate best outcomes for those within ED at end of life i.e. enable discharge to preferred place of care (community or appropriate UHP care)
  • Improve access to expert knowledge in ED, supporting patients, families and co-workers to receive rapid appropriate care planning, whilst taking into account the patient's wishes and disease staging.
  • Incorporate systems thinking, in terms of problems across localised care systems; acting on evidence to influence stakeholders to implement appropriate change.
  • Coordinate and deliver remote advice and support telephone line (for professionals) in addition to delivering peripatetic function.

  • The post holder will be required to work at a specialist level within the hospital; when lone working in the community the role requires confidence and positive engagement with patient led risk.
