Job summary
This job is to work as a Clinical Nurse Specialist within the newly developed older Adults with Cancer Service. The CNS will work alongside the Advanced Clinical Practitioner with patients aged 65 and over who are to receive systemic anti cancer therapy (SACT). The role will be a mixture of phone clinics and Face to Face, alongside regular contact with patients. The role will be working closely with all members of the Multidisciplinary team, and make appropriate referrals when necessary. The CNS will work closely not only with the ACP, but also the Oncology consultants and other site specific CNS nurses. The CNS will provide expert advice and implement patient care, and look to assess our patients using a comprehensive geriatric assessment framework. The post holder will be able to provide expert advice to our patients who phone through for advice, and ensure they receive timely interventions. The CNS will aim to provide the highest quality clinical care and advice to cancer patients This role will play a pivotal part in developing this new service.
Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS at Risk staff throughout Devon who is able to display recent relevant experience as dictated by the Person Specification.
Main duties of the job
Work with the ACP in oncology to use comprehensive geriatric assessment to assess and support older adults with cancer, pre their SACT treatments and support them throughout their treatment pathway. Manages his/her own caseload of patients whilst working as part of the multidisciplinary team. Assess new patients in the clinical area a, identifying their psychological, physiological, emotional spiritual and cultural needs in relation to their disease and proposed treatment, providing a pathway of nursing care for the patient. Undertake follow up clinics under supervision Provide clinical advice and support to health care professionals within and across organisational boundaries. To formulate individualised patient treatment care programmes in conjunction with the medical/nursing team considering
Maintain accurate patient records and ensure all relevant information is documented in medical and nursing notes
- Maintain own professional development
- Manages own caseload, and coordinates patient journey across divisions and organisational boundaries
- Works in close collaboration with ACP to ensure service meets patient's need.
- Undertakes complex assessments and review/follow up of patients and carer
- Research and Audit
- Undertakes evidence based practice and ensures dissemination of relevant research and best practice
