Job summary

We are looking for a Band 4 Macmillan Cancer Support Worker to support the Uro-oncology team in providing care for patients with urological cancers who are being treated in oncology.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

The post holder will act as a point of contact for patients and their carers, triaging calls and escalating complex and clinical concerns to the registered practitioners as appropriate. The post holder will be required to work independently, but under the guidance and supervision of the Lead Nurse for the uro-oncology clinical team to provide support and information to those affected by a cancer diagnosis. The post holder will be based within the clinical team for uro-oncology to support patients Living With and Beyond Cancer in line with the national Personalised Care in Cancer programme (PCC). They will also take a lead role in coordinating and initiating Holistic Needs Assessments, Treatment Summaries and assisting with provision of Health and Wellbeing support.

The Macmillan Cancer Support Worker will support the Uro-oncology Cancer Specialist Nurse Team based in oncology, MDTs and Mustard Tree to enhance the care and management of patients and also be required to collect data and prepare statistical reports.

  • Provide and receive complex information about patient care and interpret the patient plan for other clinical and non-clinical staff
  • A key aspect of this role involves daily and direct (eg face to face and telephone) communication with patients, relatives and carers and other health professionals. The information and nature of communication required is sensitive due to the nature of cancer and therefore communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity
  • To have a good knowledge and understanding of the Living with and beyond Cancer Pathway and understanding of cancer care and treatments available
  • To provide non-clinical advice and information using good communication skills to navigate patients through what may be a time of great anxiety and where patients may be highly distressed
  • Be aware of the individual physical and psychological needs of patients and report concerns to the registered practitioners so that any necessary treatment and/or reassurance can be given to maintain patient safety/well-being
  • Encourage and support patients in resuming activities and making healthy lifestyle choices
  • Coach patients and their carers to understand what signs and symptoms to be aware of that might indicate cause for concern and what they should do about them
  • Assist with organising and requesting extra tests /service as required
  • Following up and tracking results and acting on results in a timely, appropriate manner
  • Support the delivery of patient education through LWBC events, focus and support group
  • Provide basic advice and refer on or sign-post to other sources of support
  • Co-ordinate and organise appointments and assessments as required and to exchange that information clearly and concisely dealing with anxiety, cultural or language difficulties
  • Co-ordinate the handover with other Teams to facilitate safe and effective transition of care between services in order to provide seamless support for patients
