Job summary

An exciting opportunity has arisen within the Multiple Sclerosis team University Hospitals Plymouth to appoint an enthusiastic, motivated, dynamic nurse to make a significant difference to the health outcomes of adults with MS. We are looking to appoint a Band 7 as a developmental post allowing progression from band 6 to Band 7 on completion of agreed competencies including the completion of Non Medical Prescribing.

This is a full time Specialist Nurse Post working within an established MS team that includes three Specialist Nurses, Two Infusion Nurses, two MS Consultants and administrative staff. The team work collectively to provide comprehensive care and support for the local MS population.

The role would be required to support prescribing activity with time allocated for completion of the prescribing course as part of the role development programme.

As well as working as part of the team the successful candidate will work autonomously in the delivery of nurse-led clinics and patient monitoring.

Main duties of the job

Job Purpose:

Works Autonomously and as part of the MDT to Assess, plan, implement and evaluate expert nursing care and provide support to people living with Multiple Sclerosis and their carers. The post holder will have continuing responsibility for the co-ordination and organisation of individual treatment programmes throughout the illness trajectory and work to develop communication across divisional and organisational boundaries.

Act as an Independent Nurse Prescriber, able to distribute repeat DMT prescriptions for MS Patients with support from the MS Consultant team (not initiating new treatments) and monitoring the safety of the treatment regimes.

Act as an expert source of knowledge and information for people with Multiple Sclerosis. The post holder is the specialist who has the freedom to initiate change, using broad policies, seeking advice when necessary from the wider MDT and is guided by health, organisational and occupational policies/ guidelines. Acts as a role model for excellence in the field of Multiple Sclerosis.

Clinical:

  • Works autonomously to manage his/ her own case load of patients whilst working as part of the Multidisciplinary team.
  • Communicates highly sensitive and emotive condition related information E.g. breaking bad news to patients and relatives regarding their diagnosis of Multiple Sclerosis and prognosis.
  • To assess new patients in the clinical area, identifying their psychological, physiological, emotional, spiritual and cultural needs in relation to their disease and proposed treatment, providing a pathway of nursing care for the patient.
  • To safely conduct Non-medical prescribing practices in line with Trust Policies and Procedures to avoid delay in distributing repeat prescriptions for Disease Modifying treatments to MS patients.
  • To act as patient advocate
  • Identify Patients with MS who may benefit from disease modifying therapy.
  • Provide specialist information to support and educate people with MS (pwms) through the complex decision-making process regarding disease modifying therapies.
  • Monitor Patients with MS on treatments for side effects and efficacy, including ordering and monitoring blood results and ordering MRI scans.
  • Provides continuity of care throughout the patient journey, liaising across organisations e.g., Primary Care and social services.
  • Develops individualised programmes of care and provide highly specialised advice. Provide up to date and appropriate verbal and written information to patients and carers.
  • Provides expert clinical advice and support to and be the first point of contact for patients, carers and health care professionals. Expert knowledge is not obtainable from other areas of the multidisciplinary team. The post holder has the freedom to initiate action within broad policies seeking advice, as necessary. These are based on the post holders own interpretation of those policies.
  • Formulates individualized treatment programmes in conjunction with the medical team considering Each patient is placed on the correct treatment protocol and Investigations and treatments are organised as required.
