Job summary

The Neonatal Outreach Team has a band 6 vacancy for a neonatal nurse.

We are offering this exciting opportunity for you to be apart of the team, working between hospital and the community settings, supporting families to transition from hospital to home as well as facilitating early discharge of preterm infants.

You will be primarily hospital based and undertake lone working in the community, providing expert neonatal care for preterm infants at home. You will undertake all aspects of discharge planning for infants across the neonatal service line to facilitate safe early discharge home whilst working within the multi disciplinary team.

Main duties of the job

We are looking for a neonatal nurse with demonstrable experience to join our neonatal outreach team at UHP. The successful applicant will be:

  • Responsible for co-ordinating the nursing team in delivery of effective quality care, following a family integrated model of practice, in accordance with local, Service Line and Trust policies and guidelines with the support of the senior management team
  • Assist the team leader in the administrative management, supervision and support of a team of nurses and ancillary staff
  • Participate in Network NICU service development
  • Initiate evidence based practice and actively participate in neonatal research
  • Contribute to resource management in the provision of neonatal services
  • Engage in Clinical Governance initiatives within the Directorate
  • Co-ordinates the provision of Neonatal Outreach Services in the absence of the senior charge nurse responsible for safeguarding and family support
  • Manages the discharge process for infants requiring neonatal outreach services
  • Work within a supportive team to facilitate seamless transitions for families from hospital to home
