Job summary
An exciting opportunity has arisen to expand our Patient Flow Co-ordinator team who work within the Emergency Department of Derriford Hospital Plymouth.
We are looking for enthusiastic applicants who are organised and can use their initiative whilst being flexible to frequently changing situations, to work alongside the Senior Nurse. You will play a key role in the patient flow through the department to assist by co-ordinating admissions and discharges, ensuring TTO's and EDS's are complete and transport is arranged.We are seeking to appoint a full time, permanent position and would require applicants to possess excellent communication skills towards a range of health professionals, both internal and external, to support safe and effective admission and discharge in relation to assisting in patient flow through the hospital.
The Emergency Department is a 24-hour service and the post-holder will be expected to cover shifts which will include evenings, nights and weekends.

Main duties of the job
To work as a member of the Emergency Department team to meet the health care needs of the public, who attend with a variety of emergency medical and surgical conditions, minor injury and illness of which the Emergency Department at Derriford Hospital treat in excess of 150,000 per annum. To help maintain the overall function of the department and to ensure that clerical processes are maintained 24/7.

Patient Flow
- To liaise with the bed management, operation support, and acute care teams, as well as the medical, surgical, trauma and specialist co-ordinators to ensure that lines of communication are maintained on the 24/7 basis and that patients requiring an inpatient bed are catered for
- To liaise with the Halo to arrange patient transfers, or to facilitate specialist reviews and/or treatment, working in conjunction with the clinical teams to ensure that patient investigations are ordered and organised in a timely manner
- To actively pursue and encourage timely decision making by the nursing and medical staff and to ensure that unnecessary delays for patients are reduced as much as possible
- To ensure that the electronic patient register is constantly up to date; entering all details of the patient journey/status when they transpire
- To monitor waiting times for majors and minors patients and to alert relevant staff at key period.
- To handle internal and external telephone calls from staff, and relatives and to ensure requests are dealt with or to record accurate messages
- To communicate the status of the department regularly to senior and management staff
- To ensure all patient documentation (paper or electronic) is completed accurately before patients are admitted or discharged, ensuring that documentation is scanned in full.
- To provide practical support to the Emergency Department team when necessary to facilitate patient flow
- To undertake real-time analysis of those patients whose treatments are delayed for any reason, and to facilitate and encourage timely patient treatment using a range of different methods
- To work with minimal supervision, within Minors and Majors performing a range of clerical and administrative duties
- To support the majors and minors co-ordinators wherever possible to maintain patient flow
- To support the medical teams as much as possible in facilitating fast and effective patient care
- To contribute to and work within a safe working environment To undertake any training requirements deemed necessary by the Senior Management team; including mandatory annual training packages
