Job summary

To provide nursing care and support to children within the special school setting with complex health care needs so they can access education. This role is supported by the lead special school nurse.

The special school nursing team covers 3 special schools in Plymouth and comprises of Mill Ford, Woodlands and Cann Bridge. The team meets regularly to discuss and review our practise. This gives opportunity to develop and reflect on our service.

We work closely with our colleagues within school and with the children's community team.

Please note, pay is term time only, 39 weeks of the year, paid over 12 months.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

Provide care for children with ongoing and complex health needs within the school setting.

Provide education, advice, and training to education staff so they can assist in meeting the needs of pupils within the school.

Provide or ensure the safe provision of evidenced based clinical care in a variety of community settings.

Liaise with parents and young people regarding their health care needs in school and how to plan their care. Contribute health information for annual reviews and other assessments.

Job description

Job responsibilities

Plan and deliver nursing care to pupils within the school setting to meet their health needs.

Provide training to education staff and others on how to meet the health needs of pupils in school. Such as enteral/parental feeds, passing gastrostomy tubes, management of colostomies/ileostomies, catheterisations, tracheostomies, suctioning, emergency medications, drugs: oral, injection.

Assess plan and implement care for any pupils requiring their health needs to be met in school. Ensuring each child has a health care plan.

With support of lead school nurse undertake assessments for continuing health care, annual reviews etc .

Work as part of the special school nurse team under the lead of the Band 7 to ensure that nursing services are consistent over the 3 school sites managed by UHP.

With guidance from lead school nurse identify social care needs including protection from abuse and refer to Social Services as indicated. Produce health assessments in conjunction with the Framework for Assessment, provide reports for and attend case conferences and court as indicated.

Contribute to the maintenance of high standards of care by participating in the development of evidence-based protocols and guidelines as part of the special school nurse team.

Develop and maintain knowledge of conditions that effect children within the special schools.

Assess the need for and organise where necessary supplies of equipment that will be needed within the school setting.

Be part of the transition planning with school staff and health colleagues to support preparing for adult hood.

Attend multi-disciplinary meetings relating to pupils and provide health information as required.

Follow child protection processes to keep children safe. (Working Together 2015 )

Report any untoward incidents and complaints to the lead nurse and complete the Datix process as required. Support with clinics in school eg consultant clinics , orthoptist, dieticians etc

COMMUNICATIONS & WORKING RELATIONSHIPS

Establish methods of communication and understanding with young children or those who have sensory or communication difficulties.

Communicate condition related information to children and their families/carers at a level appropriate to their understanding.

Educate, support and guide children and their families in order to promote empowerment by facilitating an understanding of the management of the childs condition. Including educational staff so children can access school.

Supervision of untrained staff as required.

In liaison with other school/health staff refer to other agencies regarding psychology, behaviour support plans etc to meet the needs of children within the school. Use knowledge of Child Protection Guidelines to communicate sensitively on issues of child protection.

Identify appropriate self-help or voluntary organisations advising the child and family of the facilities they offer. Provide teaching and updating for education colleagues in appropriate techniques and treatments.
