Job summary

To successfully complete the Advanced Clinical Practitioner (ACP) Degree MSc (Level 7) programme in line with the trust framework for advanced practice and their primary professional regulatory body, with the expectation that on successful completion the post holder will move into a qualified ACP role at band 8a. If already qualified as an ACP you would be required to complete the renal competencies prior to uplifting to a band 8a.

As part of the ACP programme the post holder will be working towards the following and onsuccessful completion will be able to:o Utilise advanced clinical reasoning skills and assessment techniques autonomously inthe context of their specialityo Identify presenting problems and provide a diagnosiso Set appropriate treatment aims and objectives with the patiento Competently plan and implement realistic programmes of investigation and treatmento Evaluate investigations and treatment plans to assess impact and outcome.

o Relate findings to presenting pathology and revise treatment planso Demonstrate advanced clinical competence in area of speciality

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

'Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making.This is underpinned by a master's level award or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of corecapabilities and area specific clinical competence.Advanced clinical practice embodies the ability to manage clinical care in partnership with individuals, families and carers. It includes the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improveoutcomes. Job description

Job responsibilities

  1. Assess, differentially diagnose, plan, implement and evaluate individual treatment interventions for patients by virtue of expert clinical judgement and close liaison with members of the MDT.
  2. Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.
  3. Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, life style advice and care and ordering appropriate investigations, i.e ultrasounds, CXR, under the Trust IRMER training and other relevant departmental training.
  4. Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers.
  5. Ensure compliance with their respective code of professional conduct and work within their scope of practice.
  6. Works in conjunction with the MDT to clearly define values, direction and policies impacting upon care.
