Job summary

Are you ready for a challenge? Sherwood Forest Hospitals are seeking x 2 Discharge Coordinators to join our team!

The Discharge Coordinator will support the Discharge to Assess process, facilitating safe discharges from the hospital and aiding flow and capacity within the Trust.

This role requires someone who has excellent communication skills and is self motivated to be able to adapt quickly to changes with a patient and calm approach.

The role involves working across all clinical areas and will require duty at other Sherwood Forest sites (Mansfield Community Hospital and Newark Hospital) as needed.

Candidates should possess a clear understanding of the Discharge to Assess process, sound knowledge of discharge pathways, and awareness of social care and health processes.Full training will be provided for this role.

This position entails a seven-day service, including bank holidays. Current shift patterns will be a variety of 8am-4pm and 8.30am-4.30pm.
