Job summary

An exciting opportunity has arisen for an experienced MSK physiotherapy practitioner (MSK) to join our team in Mid-Nottinghamshire's integrated MSK service - MSK Together. MSK Together is a collaboration delivering joined up care pathways between primary care, MSK therapies, pain management and podiatry, rheumatology, orthopaedic and spinal services and is co-ordinated via the MSK Hub. This role is within the MSK Hub as part of the Musculoskeletal Clinical Assessment and Treatment Service (MCATS) to triage, assess, investigate, diagnose and manage complex patients.

The MSK Hub is delivered at 4 base locations, Mansfield Community Hospital, Ashfield Health and Wellbeing Centre, Kings Mill Hospital and Newark Hospital and is provided by both Nottinghamshire Healthcare and Sherwood Forest Hospitals working seamlessly together as one team.

We are looking for an individual, who is an excellent team member and is self-motivated to deliver evidence based MSK services in a friendly and supportive environment, working with wider MSK teams within the integrated service.

Main duties of the job

The post requires specialist triage, assessment and treatment skills to diagnose and manage complex Musculoskeletal cases working alongside Advanced Clinical Practitioners and consultants within the MSK Hub with a focus on quality clinical delivery. You will co-ordinate treatment plans and refer into specialist services where required, delivering personalised care and supported self-management to people living with long term MSK conditions. You will demonstrate the ability to communicate effectively with patients, consultants, GPs, and other health care professionals to improve patient outcomes.

You will be able to function well under pressure and be able to respond and adapt to changes in the current NHS environment. You will support with implementing innovative, efficient and effective ways of working within the area of MSK, developing services that are fit for purpose and which deliver against MSK Together outcomes, national targets, and patient expectations. This will ensure we are able to facilitate health gain and maximise the functional ability of patients and equitably improve the health and wellbeing of people with an MSK condition across the whole of Mid-Nottinghamshire.
