Job summary
King's Mill Hospital are looking to recruit a Diabetes Specialist Nurse to join their team of dedicated specialists for the inpatient 7 day service. This is full time permanent contract.
The ideal candidate will have excellent leadership and communication skills as well as specialist knowledge and expertise in Diabetes.
In this role you will establish and maintain your own workload, maintaining responsibility and accountability for all elements of professional nursing practise including Nurse Prescribing.
There will be a supervisory responsibility for the Diabetes Specialist Nursing Team and other non-diabetes specialist healthcare staff, together with an expectation of teaching on a regular basis for students and other health care professionals.
You will provide a specialist and advanced nursing contribution to the clinical management of patients with diabetes, working autonomously and in collaboration with multidisciplinary professionals ensuring the diabetes service is effectively coordinated, equitable, consistent and of a high standard.
The successful candidate will demonstrate a high standard of commitment and be able to demonstrate the Trust CARE values throughout their work. For the suitable applicant a band 6 development post will be considered.
Main duties of the job
To provide effective, high standard, specialist and advanced nursing contribution to the clinical management of people with diabetes to meet their complex healthcare needs, in accordance national standards, and N.I.C.E. guidelines.
Triage of patients referred urgently to the diabetes service.
To have a central role in the diabetesmulti-disciplinary team, and provide a lead specialist and advanced nursing contribution to the clinical management of patients with diabetes, working autonomously and in collaboration with multidisciplinary professionals ensuring the diabetes service is effectively coordinated, equitable, consistent and of a high standard.
This role will involve providing a seamless service for patients in Sherwood Forest Hospitals NHS Foundation trust providing diabetes services across the healthcare setting.
To provide and act as an educational resource.
- For patients/carers.
- For health professionals of all grades, including medical, nursing and allied specialities, who are caring for patients with diabetes conditions within secondary care at a local and national level.
