Job summary
The post holder will be an experienced, highly competent and confident Critical Care Outreach Nurse who is proficient in both critical care and Outreach nursing and able to utilize their clinical expertise and specialist knowledge to deliver high quality evidence/research based care and interventions to the critically ill deteriorating ward-based patient. They will be expected to be an effective leader and role model providing support, education and training to the multi-professional team caring for the deteriorating and/or complex ward patient and less experienced Critical Care Outreach Team members.
The post holder will have a significant degree of autonomy, demonstrating effective decision making skills in which they will be responsible for the co-ordination of the caseload throughout the hospital. They will review known patients, assess new patients who are considered "at risk" by members of the ward and Critical care multi-professional team and suggest management plans and deliver ward based interventions.
Main duties of the job
The CCOT service operates 24 hours a day seven days a week. With the overriding purpose is to support the provision of the highest quality patient care through personal actions and continuous improvement.'
To provide an integrated hospital wide approach to critical care
To facilitate the timely discharges from the critical care unit, providing ongoing support for patient care and management
To respond to and optimise the patients' physiological recovery, identifying those patients that would benefit from admission into the critical care environment
To provide specialised support and interventions to patients within the ward environment, supported by appropriate assessment, observation and treatment
To liaise and work collaboratively with CCOT team, CCOT lead Nurse, ACCU Matron, Nurse Educator, and ACCU leader.
To take responsibility for establishing and maintaining professional networking relationships between the ACCU and the ward environments
To provide education and training to nursing and the multi-professional team across the trust regarding critically ill ward based patients
To promote and maintain the utilisation of effective patient at risk scoring systems throughout the Trust
Co-ordinate and facilitate the appropriate transfer of patients into ACCU
