Job summary
We seek applicants for a new post of functional neurological disorder (FND) care coordinator within the South East London FND Network (FND ExSEL). This is an exciting new post to work with patients across specialist and community care environments in southeast London, as part of the implementation of a new integrated care pathway for FND. Applications will be welcomed from candidates coming from any relevant professional background, including but not limited to physiotherapy, occupational therapy, speech and language therapy, psychology, social work and nursing. Willingness to work within a multidisciplinary team is essential. Experience in FND is highly desirable. The care coordinator will be based within the FND team in the Neurology department at Denmark Hill but will liaise closely with the wider MDT both at KCH and other acute trusts and in the community.
Main duties of the job
The postholder will provide a specialist care coordination and liaison service for adults with Functional Neurological Disorder (FND) across South East London. The postholder will work autonomously to support patients, clinicians and services in managing complex FND presentations. Approximately 50% of the role will be focused on supporting acute hospitals and emergency departments, and approximately 50% on case management and follow-up of community patients not yet ready for active rehabilitation. The postholder will also contribute to the ongoing development and evaluation of the pathway. The successful applicant will have highly developed clinical expertise enabling them to plan, deliver and coordinate complex care to ensure that the needs of the person with FND and their carer(s) are best served.
