Job summary
The Patient Pathway Coordinator plays a key role in supporting the safe, timely, and efficient progression of patients through their care pathways. Working within a busy administrative team, the post holder contributes to the delivery of a high-quality, patient-focused administrative service that supports clinical teams and meets Trust and national access standards.
The role focuses on the overall coordination and oversight of patient pathways, ensuring activity is aligned with clinical priorities, Trust policies, and national targets, including RTT and cancer pathways where applicable. The post holder works proactively to support pathway performance, patient experience, and effective communication across services.
The Patient Pathway Coordinator works closely with clinicians, administrative colleagues, primary care, and patients to ensure pathways run smoothly, information is accurate, and patients are supported throughout their journey. The role requires flexibility, attention to detail, and a collaborative approach within the wider administrative team.
Main duties of the job

Act as a key point of contact for the service, communicating clearly and compassionately with patients, carers, clinicians, and the multidisciplinary team.

Manage telephone calls, emails, post, and electronic inboxes, resolving routine enquiries and escalating complex issues appropriately.

Actively monitor and manage patient pathways using the Patient Tracking List (PTL), ensuring compliance with RTT targets and the Trust's Access Policy.

Book and coordinate outpatient appointments, diagnostics, and inpatient admissions across the patient pathway using Epic.

Proactively identify delays, risks, and potential breaches, escalating and working with clinicians and managers to resolve issues.

Process referrals, maintain accurate waiting lists, and manage work queues in line with standard operating procedures.

Support clinical teams with inbox management, virtual clinics, clinic templates, and diary management as required.

Maintain high standards of data quality, ensuring patient records and demographics are accurate.

Support patients with MyChart registration and contribute to data quality and improvement workstreams.
