King's College Hospital NHS Foundation Trust in South East London is a large teaching hospital and is at the forefront of academic and nursing excellence. We are looking for a highly motivated, flexible and enthusiastic individual to recruit into a band 7 Clinical Nurse Specialist role for Paediatric Palliative Care.
As a Clinical Nurse Specialist you will work closely with the multi professional team members to enhance service delivery. This is the first post of its kind at King's, made possible by funding from the True Colours Trust. This is an exciting opportunity to shape a new service which will create a cultural shift at King's. The post holder will be instrumental in developing a coordinated approach to paediatric palliative care which will enhance the quality of life of both patients and their families. As an individual who is new to this role you will be well supported by your peers within the division and offered training and support to develop the skills required for this role. This role will enable you to gain extensive new skills in leadership and people management within the clinical environment.
Main duties of the job
As a Paediatric Clinical Care Nurse Specialist you will manage a caseload of patients with palliative care needs and participate in providing a service ensuring patients and their families are fully informed and supported.
This exciting opportunity requires a confident pioneering, dynamic children's nurse toassist in service development and adhere to evidence based clinical nursing practice in line with current research and guidelines.
The successful post-holder will build on an existing relationship with the Evelina Paediatric Palliative Care team to link with other palliative care providers and primary care to facilitate the care of this group of people in the community.

Clinical Practice/Care Delivery
- Provide specialist nursing service for babies, children, and young people with specialist palliative care needs. Provide expert nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment.
- Assess health, health related and nursing needs of the children and young people, their families and other carers by identifying and initiating appropriate steps for effective care for individuals and groups. This can include:
Assessing patients in an agreed frame of practice.
Advising on diagnostic tests.
Making and receiving referrals.
Assisting in the admission and discharge of appropriate patients within agreed protocols.
Participating in specialist clinics to meet patients palliative care needs.
Joining specialty team meetings to identify, encourage referral and promote the palliative care needs of children and young people.
- To work with the MDT to develop, implement and evaluate integrated care pathways and systems of MDT documentation, including the care of the dying person.
- Provideemotional support and work with our psychology team for ongoing advice/intervention as needed.
- Attend outpatient clinics to follow up patients and ensure all relevant information is available to enable patients to make an informed choice about their treatment.
- Ensure all patients receive appropriate follow-up and relevant information is available to enable patients to make an informed choice about their treatment and future care.
- Support and empower children and young people, their families and other carers to influence and participate in decisions concerning their care by providing information on a range of specialist health care and services.

- Promote child well-being and safeguarding them from harm, whilst working within agreed policies for reporting concerns, including concerns about colleagues.
Care Management
- To work jointly with the Evelina Paediatric Palliative Care Team, liaising effectively to ensure that the needs of the paediatric palliative care patients at Kings are met.
- Ensure all care is given in accordance with agreed protocols. To be integral in the development of and updating protocols, including working within a suitable mechanism for referrals to the Palliative Care CNS.
- Provide support and guidance to others, so that they are able to deliver a patient-focused service.
- Coordinate and attend MDT meetings.
- Ensure involvement in cross-departmental processes and planning.
- Liaise with the Paediatric Nurse Practitioner (PNP) and ward staff to ensure appropriate and timely discharge/follow-up arrangements are in place for patients.
- Contribute tothe development of appropriate patient group directives (such as symptom care plans and ambulance directives) to facilitate/enhance the discharge process.
- Ensure appropriate information and educational literature is available to help patients make informed choices as to their treatment plan and continuing management.
