Job responsibilities
To provide an expert specialist service for patients with complex needs and their care
To provide expert advice and support to patients, their families and other healthcare professionals within the inpatient setting
To ensure continuity of a high standard of evidence based nursing care, assessing health, health related and nursing needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care. This can include:
- Managing a patient caseload
- Making and receiving referrals
To accept responsibility for own patient caseload and help staff to ensure patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patients records
To be actively involved in inpatient care, liaising with ward staff to ensure high quality care
To follow-up inpatients as appropriate and ensure all relevant information is available to enable patients to make informed choices about their treatment with the involvement of carers as desired
To work as part of a team to provide a seamless service through the development of enhanced Multidisciplinary team (MDT) processes and communication
To ensure all care is given in accordance with agreed protocols
To liaise with the ward staff to ensure appropriate and timely discharge/follow-up arrangement in place for patients with appropriate liaison with their carers whilst providing advise regarding mental capacity and DOLs
To liaise with local community, social services and voluntary organisations in order to access existing services for patients, carers and health-care professionals
To complete quality audits and act on findings
To ensure all appropriate patients and carers have the opportunity to complete a This is Me document and that this is available to the patient, carers and staff
To participate at forums as Divisional, Trust or National levels to represent the service
Education and Training
To provide information for clients, carers and heath professional about all aspects of specialist care
To ensure appropriate information and educational literature is available to help patients make informed choices as to their treatment plan and continuing management developing and regularly updating literature
To regularly participate in the delivery and development of programmes of education to enhance the awareness, understanding and management of patients with dementia and delirium for all staff groups across the Trust
To work with and support ward managers and other clinical staff to identify areas where changes to practice are required. Also where opportunities exist to improve treatment and support for patients
To actively participate in educational events provided by the Trust and primary care sector
To attend, participate and contribute to meetings and conferences as appropriate ensuring that you are fully conversant with current issues both within the Trust and within the specialist services locally and national
To publish and present relevant clinical practice and service developments.
Research and Service Development.
To coordinate clinical audit (locally and national audits), in particular ensuring patient experience and carer surveys are completed and action plans developed with the wider organisational team
To take part in nursing research initiatives and support research activity within individual Divisions and also through collaboration with other agencies
To participate and assist with any Trust approved research projects conducted within the department
Develop research skills and provide support and appropriate implementation strategies for changes in clinical practice.
Leadership

To play an integral role in the operational and strategic development of the DAD service
Participate in the recruitment, selection and retention of staff
Review the effectiveness of clinical care provided ensuring that patient safety is maintained and care delivery meets the standards required by the Trust and the Care Quality Commission whilst embedding NICE Quality Standards and our commitment to the Dementia Friendly Hospital Charter
Manage change through strategic thinking, use of negotiating skills, self-awareness and effective communication. Working proactively in managing change in own speciality to improve practice and health outcomes whilst encouraging others to be innovative and adaptable in the approach to change
To actively contribute to any relevant initiatives within the Division and provide support to senior colleagues and Heads of Nursing
To act as an effective role model across the Trust
Assess and monitor risk in own and others practice, acting on results, thereby ensuring safe delivery of care
To introduce measures to actively manage and reduce risk to patients. Ensure the formal reporting and recoding of adverse incidents in line with Trust policy.
Work collaboratively with representatives of other clinical areas, disciplines and services to provide a seamless pathway of care
Participate in maintaining systems for risk assessment and minimisation including promoting a fair blame culture in order that near misses and incidents are reported and investigated
Lead and contribute to investigation of incidents and complaints when required; participate in identifying lessons learnt and the sharing of learning across the organisation
Continually monitor standards of care through quality markers, achievement of Trust targets, and benchmarking.
To be conversant with all Trust policies and procedures and ensure they are correctly implemented
Always adhere to confidentiality with sensitive personal data
To evaluate the service in terms of clinical effectiveness, clinical excellence and value for money
Actively participate in local and national networking
Personal Professional Development
To achieve a range of clinical competencies as required within the Trust and Division
Be competent in Delirium screening tools and management plans
To keep up-to-date with current literature and research in the field of dementia and delirium
To maintain your own personal and professional development
To undergo and actively participate in your own performance appraisal
To keep up to date with NMC/HCPC, relevant specialist Faculty & Trust guidelines and protocols
To adhere to the NMC/HCPC Code of Professional conduct working within and accepting responsibility for maintaining agreed levels of competence.
