Are you a highly motivated Cardiology Nurse? Do you love the challenge of improving quality and making a difference to patients and their families?
We are looking for an enthusiastic nurse to join our Inherited Cardiac Conditions team within the cardiovascular care group at Kings College Hospital. Candidates must be able to demonstrate their clinical experience in cardiology and show a passion for genetics.
The ICC service serves the patient population across South East London through the Kings Health Partners sites of Kings College Hospital, St Thomas' Hospital and Guy's Hospital. This post is mainly based at the Denmark Hill site at Kings College Hospital along with a honorary contract (Kings Health Partners passport) for Guys and St Thomas' for cross site working when needed. This post will work within a team of clinical nurse specialists, cardiologists and geneticists working with ICC patients to provide clinical triage and diagnostics, genetic assessment, and genetic counselling.
There will be teaching, clinical supervision and opportunities for enhanced clinical learning as well as developing patient assessment skills.
Successful applicants will need to demonstrate specialist skills in cardiology and/or genetics, have effective communication skills within a multidisciplinary team, as well providing support to patients and families affected by inherited cardiac conditions.
Main duties of the job
- To work alongside the team in triaging referrals appropriately to the ICC service including constructing family pedigrees to assist in genetic assessment.
- Clinically assess signs and symptoms to ensure safe and timely review and treatment of ICC patients.
- To undertake genetic assessment, genetic counselling of patients, and promote mainstreaming of genetics within healthcare.
- To assist in developing specialist local pathways and guidelines for the continuous delivery of a high-quality service for ICC patients.
- To co-ordinate the patients' multidisciplinary care whilst acting as a point of contact for patients and their families.
- To provide on-going support, education and advice for patients and their family as required.
- To liaise with the DGHs, GPs, coroner's office, and genetic services to collate and report back relevant clinical information and ensure seamless service and care of families where needed.
- To assess each patient and/ or family member for specific psychological needs, linking them to relevant ongoing sources of support eg support groups, counselling, social services etc.
- To liaise with appropriate patient support groups and attending support groups as an expert speaker.
- To introduce measures to actively manage and minimise clinical risk to patients. To ensure the formal reporting and recording of any adverse or untoward incidents, in line with the Trust Policy.
