Job summary
An exciting new opportunity has arisen for an enthusiastic, dynamic and motivated pharmacist to join the Post-Acute Medicine team. You will be responsible for delivering high quality clinical care to patients across Post-Acute Medicine at the Denmark Hill site. You will have the opportunity to work across a broad range of specialties within Post-Acute Medicine which includes respiratory, Cystic Fibrosis, stroke, gastroenterology, diabetes and elderly care.
The successful candidate will also work as part of a dedicated project team, working to improve the discharge process from medical wards through clinical audit and quality improvement initiatives. Workflows will include availability of medicines on wards, the counselling of patients and/or their carers in an appropriate format to ensure safe and effective medicines use on discharge, ensuring the safe & effective transfer of care and utilisation of the Discharge Medicines Service.
Main duties of the job
- To ensure the safe, effective and economical use of drugs for all patients in the Trust.
- To complete all documentation according to the trust and departmental policies.
- To ensure that all documentation is processed and filed correctly.
- To be accountable for own professional actions, but to be guided by national protocols, KCH Formulary and other local policies and procedures in relation to the provision of a clinical service.
- To ensure services provided are in line with local and national policies, guidelines and frameworks.
- To support specific service initiatives and changes affecting the clinical pharmacy service in the department and across the Trust.
- To support the implementation and audit of plans, guidelines, procedures, policies and service changes.
- To actively encourage the adherence to all procedures relating to the clinical pharmacy service and to suggest changes in practice and procedures as appropriate.
