Job summary
An exciting opportunity has arisen for a highly motivated, caring and enthusiastic practitioner with a significant experience at band 6 as an adult nurse, occupational therapist or a mental health nurse to join our dedicated Dementia and Delirium (DAD) Team. The post holder will have previous hands on experience of caring for patients with dementia and/ or delirium and supporting their carers. In addition to supporting staff on the wards to care for people with dementia and delirium you will take the lead to develop and deliver DAD training across the site to all members of staff.
Main duties of the job
- The overall job purpose of the Dementia and Delirium Clinical Nurse Specialist / AHP Specialist is to act as an expert resource for ward teams / departments in order to support them in the delivery of effective care for patients with dementia and delirium.
- As a leader s/he will be responsible for ensuring a good understanding of dementia and delirium throughout the organisation, to ensure patients and carers receive a high standard of care.
- The post holder will lead on the delivery of dementia and delirium teaching and development to nursing and medical, care support staff and AHPs at the Denmark Hill site of Kings College Hospital. The post holder will take primary responsibility (with the support of the DAD team) for providing a progressive learning environment for all grades of nursing / care staff and support the development of staff members who wish to develop specialist skills in caring

Clinical Practice
- Advise on the care of people with dementia +/- delirium and their carers/families with complex care needs from peri diagnosis to bereavement support.
- Improve identification and management of co morbidities and frailty for people with dementia +/- delirium.
- To accept responsibility for own patient caseload, make and receive referrals and help staff to ensure patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patients records and hand held records
- To support staff in delivering person-centred care plans for people with dementia including positive risk management (e.g. falls, delirium, depression, incontinence)
- To provide staff with knowledge and training related to a range of psychosocial interventions and emotional support to promote health and wellbeing for people with dementia and their carers/families.
- Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia.
- Ensure that all legal requirements relating to mental capacity, safeguarding. DoLS and mental health are considered and adhered to in relation to patient care.
- Act as an advocate for people with dementia and their carers/families to promote their rights and best interests.
- Participate in case conferences, discharge planning, continuing healthcare process and best interest meetings, where required.
- Work collaboratively with health, social and voluntary sector services to contribute to coordinated care, both within the hospital setting and across the acute-community pathway.
- Manage care transitions and support effective discharge for people with dementia and their carers/families in order to reduce frequency of admissions and length of stay.
- Ensure that care is delivered in accordance with relevant local and national guidelines and all relevant Trust policies.
- Recognise the limits of own competency and professional boundaries and make appropriate and timely referrals to other services where required.
EDUCATION
- To provide information for clients, carers and heath professional about all aspects of specialist care
- To ensure appropriate information and educational literature is available to help patients make informed choices as to their treatment plan and continuing management developing and regularly updating literature
- To lead and develop programmes of education to enhance the awareness, understanding and management of patients with dementia and delirium for all staff groups across the Trust.
- To work with and support ward managers and other clinical staff to identify areas where changes to practice are required. Also where opportunities exist to improve treatment and support for patients
- To actively participate in educational events provided by the Trust and Integrated Care System
- To attend meetings and conferences as appropriate ensuring that you are fully conversant with current issues both within the Trust and within the specialist services locally and nationally
RESEARCH
- Lead on and support the audit and evaluation of quality improvements and initiatives aimed at improving the delivery of dementia care within the acute sector of the Trust.
- To take part in nursing research initiatives and support research activity within the ASM and other care groups and collaboration with other agencies
- To co-ordinate clinical audit, in particular ensuring patient experience surveys are completed and action plans developed
- Be actively involved in the completion of the National Audit for Dementia (NAD)
- Undertake analysis and evaluation of the dementia services within the Trust and make any recommendations to the relevant committees
- Be responsible for ensuring that a variety of learning opportunities are available to support trust staff to enhance their knowledge and skills in relation to dementia and delirium.
- Work alongside teams, acting as a role model, supervisor and support resource to ensure all staff members are facilitated in their learning and care delivery with regards to dementia and delirium
- To publish and present relevant clinical practice and service developments.
- MANAGEMENT
- To play an integral role in the operational and strategic development of the DAD service
- To work with the team to develop policies and protocols to improve patient care
- To liaise effectively with all other disciplines within in the Trust and at local level
