Job summary
The post holder will be a skilled and committed renal nurse with experience working with people with advanced CKD who are frail or have chosen the supportive care pathway.
They will have advanced clinical skills and be able to manage their own patient caseload with a high degree of autonomy.
They will have experience of managing or leading a team, driving change, and adapting to changing service needs.
Main duties of the job
- To work within the Care Group to provide, lead and develop a comprehensive patient-centred, nurse-led renal frailty and supportive care service.
- To work autonomously and effectively deliver a nursing service of the highest quality to patients who have chosen supportive care; those who are living with frailty on renal replacement therapy; and those who choose to withdraw from dialysis.
- To provide clinical, management and professional leadership to nursing staff at all levels across the department.
- To develop communication and collaborative working with local geriatric and palliative care services in secondary and primary care and other community services for older people.
- To be a specialist resource in supportive/elderly care and frailty providing high quality comprehensive advice, information, education and support to clinical teams, patients, carers and families.
- To autonomously manage a case-load of renal patients and provide comprehensive nursing care to those with frailty, supportive, palliative and end of life care needs, ensuring that patients are fully informed and supported by providing clinical and social support.
- To establish clear lines of communication between teams to ensure cohesive multidisciplinary management of patients.
- To take a role in research, audit and measuring quality indicators within the specialty.
- To develop all staff within the multidisciplinary team, providing support, advice, formal and informal education.
