Job summary
The open access stratified follow-up coordinator in Breast Care will have specific responsibility for establishing and maintaining robust systems for collecting and analysing clinical information on breast cancer patients. This will involve being familiar with the various trust and cancer information systems, and being able to enter, extract and manipulate data.
The post holder will use existing cancer information systems to efficiently tracking patients on follow up pathway and ensuring timely and accurate collection of data, recall and investigation scheduling. The post holder will also be required to produce reports and undertake audits as required.
They will act as the key support for the Breast CNS's nurse led clinics organisation for Stratified follow up and Remote Monitoring, including booking and out-coming clinics, sending treatment summary record and contributing to identifying delays or cancellations of investigations which may result in delay or lost to follow up care
Main duties of the job
Help coordinate the patients who are going on to open access stratified follow up or remote monitoring follow up care for breast cancer patients
Book all patients appropriately on to the database/Somerset after clinical identification at the Multi-Professional team using standard best practice protocol of stratified follow up care
Track all breast cancer patients who are on stratified follow up. Yearly mammograms, and/or MRI'S, and Ultrasound scans are being managed and reviewed. To raise cancer follow-up surveillance results to the clinician so a letter can be sent to the patient and their GP in a timely manner.
They will act as the key support for the Breast CNS's nurse led clinics organisation for Stratified follow up and Remote Monitoring, including booking and outcoming clinics, sending treatment summary record and contributing to identifying delays or cancellations of investigations which may result in delay or lost to follow up care
Provide support to the Breast CNSs with all elements of the Stratified Follow up and remote monitoring care (including initiating eHNA assessment checklist at point of Treatment summary consultation; at progression or recurrence of cancer)
This is a role with lots of variety and ad hoc demands and the post-holder must possess excellent time management and planning skills. This post may develop over time to meet the specific needs of the clinical unit and the changing demands on the service.

To have a full understanding of the Recovery Package, Holistic Needs Assessments and care plan, Treatment Summaries, Health and Wellbeing events and Stratified Follow up.
To be responsible for the recording and management of the stratified follow up database and ensure accurate data collection to support the process.
To develop a process for Treatment Summary, post treatment HNAs and HWBE completed reporting.
To use hospital information systems to record and review patient information of both clinical and non-clinical nature.
To act as a key liaison point between support services within the pathway.
To analyse data and produce ad hoc reports as directed.

To attend meetings as required, taking and writing up notes as requested.

To undertake further duties (deemed appropriate to the grade and responsibility of the post) not covered in the job description to ensure the smooth running of the department.
To undertake all administrative work for the Breast Cancer CNSs team in relation to Personalised care related to Treatment Summaries; Stratified Follow up Care and Remote Monitoring care ensuring high levels of accuracy and proficiency in documentation and communication.

To refer patient queries onto either the Breast CNS or one of the appropriate medical/surgical teams and to provide appropriate contact details as required.
