Job summary
We are looking to recruit a band 5 Pathway Coordinator to join us in Neuro Rehabilitation at the Orpington Site.
The post-holder will work as part of a multidisciplinary team (MDT) to coordinate the administrative pathway of patients within Neuro-Rehabilitation for two wards at Orpington Hospital
Main duties of the job
- Responsible for ensuring the discharge pathway for the Ontario 2b and Frank Cooksey Rehabilitation Unit patients is supported and delivered by providing a seamless service from admission to discharge.
- Central point for all internal and external referrals to the Ontario 2b and Frank Cooksey Rehabilitation Units. Attendance at weekly referral meetings with a responsibility to compile, circulate and update the Assessment positioning list on a weekly basis.
- Supporting the MDT Team to ensure that there is effective communication between disciplines, social services and patients.

ADMINISTRATIVE SUPPORT:
- As part of the MDT, liaise with the team, internal and external partners, other trusts and service providers to ensure patient flow throughout the unit;
- Minute meetings: Performance and Governance and family meetings;
- Prepare and participate in the weekly multi-disciplinary discharge planning meetings and following through on action points, ensuring completion to agreed timescales;
- Maintain the electronic MDM list and update twice weekly as progressions are made during the patient discharge pathway;
- Support the team in submitting community neuro and physical disability referrals for patients requiring specialist brain injury and YPD services;
- Participate in investigating any counter fraud and overseas entitlement issues;
- To act as a resource person within on non-clinical issues to provide accurate information, advice and support to the multidisciplinary team, patients, carers and general practitioners.

- Capturing data and transfer of care activity from the unit.
- Ensure that the data on electronic patient records (EPR) is correct and reflective of the patient stay.

- Maintain expected dates of discharge electronically using the EPR function.
