Junior Sister/Charge Nurse - Medicine KOPAU

Job summary

You will have worked for one year in a Band 5 role. As a KOPAU nurse, you will have an interest in working with patients who are 65 and above, sometimes frail with various co-morbidities. You will need to be passionate about providing a high quality of care and happy to be a patient advocate.

You will also need to be dynamic, able to think on your feet and willing to work in collaboration with the multidisciplinary team to assist in supported discharges and admission prevention

You will need to lead the team in the absence of the Unit Manager.

Main duties of the job

Demonstrate ability to work autonomously but also as part of a multidisciplinary team and be able to proactively manage your own caseload.

Passionate about providing a high quality holistic care to patients

Will need to be knowledgeable or have a special interest in the co-morbidities of patients that are over 65 years.

Patience, empathy and excellent communication skills are key

Must be able to carry out phlebotomy and venepuncture or be willing to be trained to develop this skill

Will maintain compliance with Trust/local policies and guidelines in regards to infection control, and participate in benchmark and quality audits.

Carrying out appraisals, manage Healthroster and sickness in the absence of the Unit Manager.

Support, train and manage the administrative and clinical team.

Maintain the well-being ethos of the unit.

CLINICAL PRACTICE

To adhere to the NMC Code of Professional Conduct and adhere to this at all times.

To demonstrate clinical expertise in the care of any patients receiving treatment in the unit

To accurately assimilate and interpret clinical information about the patients condition, promptly report changes and instigate remedial action as appropriate.

To act as a resource person to others, providing accurate information, advice and support to the multidisciplinary team, patients and their relatives.

To ensure that accurate records are maintained reflecting both changes in the patients condition and the treatment delivered.

To ensure that patients and their relatives are updated regularly with accurate information, and ensure that this is presented in a way that they understand.

To ensure that Clinical Guidelines are implemented and to be actively involved in auditing practice in conjunction with Line Manager and practice development staff.

To ensure that practice is evidence based and with support assist in the development of policies and protocols within the unit

Education and Research

  • Work collaboratively with the practice development team, Consultant nurse, matron and clinical nurse specialists to ensure staff receive appropriate education and staff development
  • Ensures the development of provision of a good quality clinical learning environment that supports pre-registration nursing students and continuing professional development
  • Take part in external to the unit education sessions particularly supporting and promoting the use of KOPAU and admission avoidance

Management

  • Facilitate and supports nurses in working collaboratively with all other members of the multi-disciplinary team.
  • Manages the duty roster and negotiates skill mix levels ensuring that they most appropriately meet service needs
  • To review and process day treatment and HOT referrals to the service on a daily basis ensuring all information is present and the relevant multi-disciplinary team members are fully informed.
  • Works closely with the Discharge Suite staff to support patients in their use of Hospital Transport
  • Keep the unit manager informed of any untoward incidents and complaints
  • Work with the Human Resources Manager on the resolution of routine HR issues
  • Discuss with unit manager any plans in relation to service development, service improvement and the resolution of any on-going problem
  • Undertake some of the unit staffs appraisals ensuring they have a Personal Development Plan
