Job summary
The Flow Navigator provides support to improve the timely assessment, treatment, decision making and admission or discharge for patients present in Emergency Department (ED). The role will focus on patients pathway and support provision of the 4 hours target and prevent where possible, 12 hours beach.
**9-The overall aim of the role is promote optimal care by monitoring the patient's journey through the Emergency Department highlighting, escalating and resolving blockages to the relevant pathway with the support of key personnel.
The Flow Navigator will be required to have clear understanding of all ED pathways and support any new pathway developments and be familiar with all ED key performance indicators and escalation protocols, to be able to support the trust in achieving the required level of performance in relation to performance standards.
Main duties of the job
Main Duties and Responsibilities:
Support of patient flow through the department.
- Escalate any delays in line with agreed pathways and trust inter-professional standards document via the appropriate senior staff
- Maintain a robust and frequent reporting l
- Ensure that the physical flow of patients is not compromised by staffing resource by liaising with head porter and by providing manual support .
- Review of DTAs ongoingly to ensure reviews and decisions as well as any specific care requirements take place or are highlighted
- Escalate ambulance triage times going over 15 minutes to NIC and Bronze on-call.
- Highlight potential breaches coming up within 45mins to NIC and EPIC, escalate
- Monitor the needs of patients who required admission in relation to specialist beds
- Document all delays and maintain accurate and comprehensive records to support, review and audit.
- Record live breach validation
- Keep up to date with relevant performance and operational reports.
- Participate in audit activity as directed and be able to provide an end of shift report daily.
- Prepare and present accurate data and highlight any issues within ED at the regular daily ED huddles
- Ensure that incidents are raised and reported in timely manner and incident report completed as per Trust policy.
