Job summary

The cardiorespiratory liaison team was developed 4 years ago as a Health Foundation supported project '3 Dimensions for Long-term Conditions (3DLC)'. The team provides integrated mind and body health care for patients with long-term medical conditions (LTCs).

It is a genuinely collaborative venture, bringing together mental and physical healthcare and research and innovative practice, using a generalisable model of integrated mind/body health care pathways for people with long-term medical conditions, specifically, heart failure, and COPD. Having demonstrated successful outcomes during its pilot the team are now sustainably embedded in the Departments of Respiratory Medicine and Cardiovascular Medicine at King's College Hospital.

Main duties of the job

We are seeking to recruit a Social Worker for the Respiratory Team at King's College Hospital. The post holder will work closely with the multi-disciplinary team and will provide a Social Work service to adult respiratory patients, their families and carers. This is a part time role and will provide inpatient and outpatient support and work as part of a specialist multi-disciplinary team.

This post is a substantive Social Work role that aims to build on the success of existing psycho-social support for patients.

The Adult Respiratory service delivers a model that integrates medical, social and psychological care with the aim of improving wellbeing and clinical management in complex cases who are most at risk of their condition deteriorating. This Social Work position will be joining a well established multi-professional team and will involve ward and community work. Flexible working is expected including opportunities to work remotely.

The principle underpinning the team's delivery model is that integrating medical, psychological and social care leads to better outcomes related to quality and value. The service aims to achieve this by supporting an integrated care delivery system that encompasses commissioned social and psychological support in conjunction with the delivery of primary, community and secondary care in an inner-city population with high levels of Socio-Economic deprivation leading to health inequalities.
