Patient Pathway Tracking and Support Coordinator

Job summary

Working Full time: 37.5 hours per week.

The post-holder is responsible for providing support to the service team within Planned care including the tracking of the PTL and other operational duties.

This role will require strong effective working relations with relevant staff, along with colleagues from the divisions and support services.

Main duties of the job

The post-holder is responsible for providing support to the service team within Planned care including the tracking of the PTL and other operational support duties. Provide advice and expertise to the Patient Pathway team to support the tracking of elective care pathways and validation of data entered onto the trusts Electronic Patient Record (EPR) in line with 18 week national rules and Access Policy.

To understand 18 week referral to treatment (RTT) rules and other related service targets and use them to manage all elective pathways.To ensure validation and pathway tracking SOPs are developed, updated and followed by the team. To deal with telephone and electronic enquires from external and internal sources as appropriate.

Carry out audits of key validation/pathway tracking tasks against national standards highlighting possible issues which may be incorporated into work programmes. Monitor local metrics and standards, and initiate any necessary improvement. Identify and amend incorrect data entered on the trust's EPR by using agreed correction methods and complex fixes.

To support the team in the pro-active tracking of patient pathways and to ensure any concerns relating to treatment schedules and potential breaches are communicated to the relevant staff in order to be resolved. Assist the Service Manager in the production of performance dashboards and reports as required. Assist the Service Manager to monitor / resolve jobs on the patient pathway work plan.

o The Countess of Chester Hospital: providing 473 general and acute beds.o Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility.o Tarporley War Memorial Hospital: a base for community services which serve the local rural population.
