Job summary

Working Part time: 6 Sessions per week

The post holder will provide high-quality medical care to patients managed within the Trust's Hospital at Home service. This will include undertaking Urgent Community Response visits (2-hour UCR) and reviewing patients receiving ongoing care on Virtual Wards.

The role supports admission avoidance, early supported discharge, and rehabilitation closer to home, working as part of a multidisciplinary team (MDT) alongside advanced nurse practitioners, therapists, pharmacists, and community nursing teams.

The post holder will also have up to 2 sessions to provide senior medical oversight to patients admitted to Bluebell Ward at Ellesmere Port Hospital. This is a 26-bedded community rehabilitation ward, caring for patients who are mostly medically optimised and awaiting discharge or receiving therapy. This should include a once-a-week ward round and is supported by a full-time ward based Advanced Nurse Practitioner.

Main duties of the job

1. Manage the medical requirements and provide medical oversight to patients being cared for by the intermediate care service (Hospital At Home). Supporting the multi-disciplinary team and responding to requests for advice and assistance from the service reception, administration and nursing staff.

2. Manage the medical requirement and provide medical oversight to patients being cared for on Bluebell Ward at Ellesmere Port Hospital in conjunction with the ward based ANP team, comprising of a weekly ward round and advice to the ANP when required during working hours.
