Job summary
Working Full time: 37.5 hours per week
We are looking for a Rehab Assistant to work within the Rapid Response Team, providing an early supported discharge service to patients from The Countess of Chester Hospital & Ellesmere Port Hospital.
You will be required to work shifts over 7 days between the hours of 8am and 9pm.
You will be lone working across the West Cheshire Community. A full driving licence and car are required for this role.
Main duties of the job
- The post involves assisting patients with their activities of daily living, providing resettlement, rehabilitation & care to patients in their own home, delivered in accordance to the patient's Care Plan.
- The post holder will support therapists and nurses with their patient caseload, will manage a caseload of your own under the supervision of qualified staff & Senior Community Support Workers, and have general administrative and housekeeping tasks as required to facilitate the smooth running of the service.
- The post holder must have excellent interpersonal skills, be self-motivated, empathetic and able to develop practical skills. It is essential to have a holistic view to patient care, with focus on facilitating patient safety & independence in their own home.
- The Rapid Response Team provides a service 7 days per week including bank holidays and evening shifts.
