Working Part time : 30 hours per week
This post is to provide the Haematology transfusion clinic and to support patients with Haematology requirements and their relatives and carers from diagnosis throughout their journey.
The post holder will act as a Key Worker and a specialist resource of current clinical information addressing the specific needs of individual patients through direct/indirect interventions.
The post holder will possess a critical understanding of detailed theoretical and practical knowledge in their specialist area of practice.
They demonstrate initiative and are creative in finding solutions to problems and have some responsibility for team performance and service development. They consistently undertake self-development and liaise across all care settings, referring patients to other relevant Health Care Professionals, when appropriate.
Main duties of the job
To manage a caseload of patients through the transfusion clinic assessing their needs and requirements and providing and following a planned treatment plan.
Liaise with the multidisciplinary team both within the organisation and the community with regards to current and future treatment and refer on for potential support required.
You will ensure that the service you provide is evidence based acknowledging the need to consider service improvement in order to ensure quality service delivery.
Support the ward manager in the education needs of staff surrounding transfusion and its safe delivery
You will establish and maintain effective communication with patients, relatives and carers to always ensure their involvement in the decisions about treatment, planning and the delivery of their care.
You must have excellent leadership qualities and be able to act as a role model for the Nursing Team to ensure the highest standards of care and patient safety.

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and to Welsh patients covered by Betsi Cadwaladr University Health Board.
