We are seeking a dedicated and experienced Anticoagulation Specialist Nurse to join our team at Countess of Chester Hospital NHS Foundation Trust. This is a pivotal clinical role focused on delivering safe, effective, and evidence-based care for patients requiring anticoagulation therapy.
The Anticoagulation specialist nurse plays a pivotal role within the hospital trust, managing the delivery of high quality, patient centred anticoagulation services.
This clinical role involves the assessment, initiation, monitoring and ongoing management of patients on anticoagulation therapy. They will be responsible for the complex case management of anticoagulation planning, bridging therapy, and management in cases of complexity in high-risk patients (e.g. renal impairment, cancer associated thrombosis, or atrial fibrillation).

Main duties of the job
The role requires effective working with consultants, pharmacists, haematology, cardiology, and primary care colleagues to ensure seamless patient care across the pathway. They will also contribute to clinical governance, service improvement, and training initiatives within the trust, ensuring compliance with local and national guidelines.
Key responsibilities:
o Running Anticoagulation clinics.
o Providing expert clinical assessments and anticoagulation dose adjustments.
o Educating patients and staff on anticoagulation therapy and safety.
o Managing audits, conducting risk assessments, and participation of incident reviews.
o Supporting the implementation of new guidelines and digital systems (INR monitoring tools).
o To participate and support the Anticoagulation nurse manager with all mandatory team training and compliances, referring timely to occupational health, human resources with appropriate senior escalation and involvement, with support.
