The Countess of Chester Hospital NHS Trust is host to the Therapies and Integrated Community Services which includes the Hospital at Home service, acute rehabilitation, integrated discharge and Rapid Response. They are all unique in providing a different focus on care, whilst ensuring a person centered approach with the aim of optimising physical health and wellbeing to the population of Western Cheshire. The Hospital at Home service aims to prevent unnecessary hospital admissions whilst also expediting IV discharges from the acute service.
An exciting opportunity has arisen within the Hospital at Home team for an enthusiastic, highly motivated Registered Nurse preferably (but not essential as training can be given) with experience in Intravenous Drug administration to support patients in remaining out of or be discharged early from the acute hospital.
Main duties of the job
The successful candidate would be expected to work within the team delivering both IV medications and along with other elements of patient centered care within the patient's own home.
The post is intended to provide improved outcomes for our patients by providing safe, kind and effective care.
