Integrated Discharge Sister - Band 6 Job Advert
We have an exciting opportunity within the Integrated Discharge Team for a band 6 IDT Sister.
The Integrated Discharge Team are based in Moston lodge, this post is key to facilitating complex discharges from the Acute Trust.
The successful candidate will be supported by IDT Manager and all necessary training provided. We are looking for an organised, professional and enthusiastic member of staff who is looking to further their career within the Countess of Chester Hospital.
Main duties of the job
To work as an autonomous practitioner to perform a specialist nursing assessment patients. To use clinical reasoning to deliver and adapt individualised discharge plans, involving patients and carers/relatives in the planning and delivery.
To be professionally responsible for managing a defined caseload of complex cases and lead Discharge Planning/Best Interest Meetings and Professionals meetings as required.
To facilitate Red and Green Multidisciplinary Meetings daily on the wards to ensure active discharge planning is being undertaken to progress the patient to their date of discharge.
To identify which discharge pathway the patient requires and in conjunction with the ward staff ensure this is documented on tele tracking at the daily meetings.
To provide education/training to the ward staff and other members of the multidisciplinary team in all matters related to discharge planning.
To act as a point of contact for patients/carers and others for information on the discharge process and NHS continuing health care, assisting patients to access advocacy schemes and external advice when necessary
