Job summary

Main duties of the job

1. Be accountable for performing regular assessments on patient needs, to plan, implement and evaluate care delivery and be adaptable to changing health care requirements.2. You will be responsible for establishing and maintaining effective communication with patients, relatives and carers to always ensure their involvement in the decisions about treatment, planning and the delivery of their care.3. You will take responsibility for working collaboratively with your team, other professionals and agencies to always ensure the patient's needs are met, especially in relation to their on-going health care needs, their transfer and their discharge planning.4. Be accountable for ensuring your daily practice fulfils your professional requirements as outlined by the NMC code of conduct and guiding documents.5. Be accountable for maintaining your active status on the NMC register6. You will actively participate in patient and public involvement activities7. All employees have the responsibility to comply with the Trusts Infection Control and Prevention policies and procedures at all times. Strict adherence to effective hand hygiene is essential.

The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital.The Trust provides services in all of the surgical and medical specialties common to a District General Hospital with some specialist services. The Trust serves the whole of the local population currently in excess of 250,000 residents which geographically covers populations in England and North Wales.The Trust strives to continuously improve and sustain the quality of its services for patients, providing care to more than 445,000 patients per year, with almost 4,000 staff working together putting quality and patient safety, at the heart of everything we do.   • Mentorship & Assessing qualification (or acquired following Preceptorship & after 12 months in post)
  • Newly Qualified Nurses - Will complete the Trust's Preceptorship programme within the first 6 months of contract

Desirable

  • Post-registration qualification or evidence of further study
  • Relevant speciality qualification (may be required for certain practice areas e.g. Critical Care)

Knowledge and Experience

Essential

  • Development of speciality knowledge
  • Basic financial awareness
  • Basic IT skills in word processing/Hospital systems
  • Understanding of quality change in a clinical setting
  • Basic knowledge of professional & NHS issues
