Job summary

Are you passionate about high quality Stroke and TIA care and looking for a new challenge, with the opportunity to build on existing clinical skills? An excellent opportunity has arisen for a Stroke Coordinator to join our team.

We are part of a Hyper Acute / Acute Stroke and Stroke Rehabilitation Service covering West Cheshire; a population of around 250,000. We work as part of the Cheshire Merseyside Stroke Network, with close links with The Walton Centre. We are one of only a handful of Hyper -Acute Stroke Research Centre teams working out of a District General Hospital.

You will be part of a friendly skilled nursing team providing: specialised assessment of stroke patients; acute monitoring; and take part of hyper acute treatments - thrombolysis and thrombectomy. You will work closely with the multidisciplinary team from all stroke ward and therapy areas, and be part of the Hyper-acute and Stroke Clinical research team.

You will be given the opportunity to take part and carry out new projects and data collection to optimise patient pathway, and patient experience. Previous nursing experience in stroke and/or acute setting is preferred along with good communication skills and the ability to work effectively as part of a multidisciplinary team striving to deliver and maintain a high standard of patient care at all times.

The successful candidate will be supported as part of an agreed individual development pathway with key skills and knowledge within specialty.

Main duties of the job

  • To review all confirmed and suspected stroke patients within the ED departments, assessing, diagnosing, requesting radiological imagery and making clinical decisions alongside the stroke medical team.
  • Assisting medical staff to thombolyse suitable stroke patients and other radiological procedures.
  • To manage stroke patient flow from ED to the stroke unit ensuring appropriate placement of all patients.
  • Be responsible for the meeting the key stroke targets: swallow, admission and imaging and treatment targets for stroke patients.
  • Identify suitable patients for recruitment to stroke research studies.
  • Assist ward with patient repatriation to other hospitals.
  • Assist with data collect / discharge plan on the wards when not assessing patients.
