An exciting opportunity has arisen for an Advanced Clinical Specialist Physiotherapist - Falls Lead within community.
The role is a part time position targeting patients who have fallen or are at risk of falling, but currently do not meet referral criteria for services such as community rehabilitation and therefore, would not routinely receive a multifactorial falls assessment.
Hours per week: 17 hours
Main duties of the job
The role will work collaboratively with the Falls Lead in Mid Cheshire to continue project based work, commenced by the existing post holder. The role will also offer Advanced Clinical guidance to care community therapists across East Cheshire and will work closely with multi disciplinary team colleagues to offer falls training and support to care homes across East Cheshire to improve quality care.
The role strives to embed falls pathways across East Cheshire Care communities, with a continual drive for service development and quality improvement.
