The Macclesfield Care Community in East Cheshire NHS Trust are looking for experienced Community Staff Nurses to undertake a 12 month secondment to work towards becoming a Community Caseload Holder.
Successful candidates will be supported in clinical practice by SPC Practice Assessors to work towards the competencies required to become effective Community Caseload Holders and apply for the Specialist Practitioner programme for District Nursing.
We're based at Waters Green Medical Centre. Our caseload consists of housebound patients and we cover a wide, semi-rural patch consisting of the market town and surrounding countryside.
This is an exciting opportunity to join our innovative team to drive forward the expansion and modernisation of community nursing service with commitment to deliver the NHS Long Term Plan. The successful candidate will work closely with the therapy team, ACPs and GPs within the centre. Travel is an essential part of this role. Candidates who don't currently have the V150 prescribing qualification may be considered and are encouraged to apply.

To provide holistic, high quality, safe and skilled health and social care / support to patients, carers and their families
To work closely with members of the Integrated Care Team to provide rapid and planned expert professional assessment and treatment interventions for patients in their own homes or other community settings, putting patients at the centre of their care and enabling patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible
To be responsible /manage individual and team caseload of patients and carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of care programmes.
To provide leadership and support across the care community team demonstrating emotional intelligence to recognise pressures of self and team
