We are seeking a compassionate and dedicatedCommunity Clinical Support Worker to join our community care team. You will support patients in their homes and community settings, helping them maintain independence, dignity, and quality of life. Hours: 15 Hours per week

Main duties of the job
Assisting qualified nurses to provide safe and effective care in the community for housebound patients and to assist with procedures in the nurse led clinic if required.
Carry out wound care and dressing changes under the direction of a qualified nurse.
Support the nurses responding to crisis visits and other urgent patient needs.
Observe, record and report changes in patients' condition.
Assist with administration tasks as required and maintain an accurate appointment system.
Support patients and their families, offering reassurance and good communication.
Promote patient independence and patient dignity at all times.
