Bollington, Disley, Poynton Team in East Cheshire NHS Trust is looking for an experienced community nurse who has the enthusiasm and positive approach to join a multi-disciplinary team with a vision to deliver the right care, at the right time, to help improve the lives of our local residents.
We actively promote personal development with a competency-based framework which will be provided to ensure support to successful individual.
The post holder will work within the Community Nursing teams of East Cheshire, in collaboration with colleagues in Therapies and Social Services, as part of multi-professional working.

Main duties of the job
To deliver physical, psychological and emotional care to patients in line with the patient care plan and under the direction and supervision of the registered case manager in the patient's home or community-based facility.
To report information to trained staff on progress of patients and any variance/change in patient/carers care management in a timely manner. To complete patient records in line with local record keeping standards and protocols
To use IT systems for relevant tasks e.g. accessing patient records and results, completing record keeping.
To maintain a patient orientated approach towards care delivery, promoting a rehabilitative and self-care approach
To put person centred values into practice and to promote dignity in your day to day work with the individuals you support
To follow the Trust guidance in moving and handling and comply with the patient's care plan.
To report any issues that may put health, safety and security at risk following the Trust Risk Assessment process.
To report accidents/ incidents of patients or self on the Trust reporting system (datix)
To report any actual or potential abuse or concerns to the Team Lead and to summon help immediately in a crisis situation.
To report to manager/trained nurse any untoward incidences (e.g. complaints, accidents, defective equipment) and take appropriate action to ensure safety of patients, staff and relatives.
