The Macclesfield Care Community team in East Cheshire NHS Trust is looking for a Community Caseload Manager who has the enthusiasm and positive approach to join a multi-disciplinary team.
This role is to support our High Intensity User caseload identified through the Care Community High Intensity User programme (supported via the Better Care Fund). As the District Nurse High Intensity User lead, you will work in partnership our primary care colleagues to support our patient population in receiving the required interventions to enable self care and ongoing support.
The role involves looking at a patient's core reasons for high use of services, and working with them (and their families) to develop a person-centred approach to better manage their health and wider wellbeing concerns to improve their quality of life and reduce their need for emergency contacts with services. This will involve collaboration with wider systems partners, support a multi-disciplinary approach to patient care.

Main duties of the job

To provide holistic, high quality, safe and skilled health and social care / support to patients, carers and their families
To work closely with member of the integrated care team to provide rapid and planned expert professional assessment and treatment interventions for patients in their own homes or other community settings, putting patients at the centre of their care and enabling patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible
To be responsible /manage individual and team caseload of patients and carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of care programmes.
To provide leadership and support across the care community team demonstrating emotional intelligence to recognise pressures of self and team
