The purpose of this role is to assist in the delivery of a comprehensive, high quality cardiac rehabilitation service.
The post holder will support in the provision, of secondary prevention, education and exercise supervision to those patients who have experienced a Myocardial Infarction, PCI, heart surgery or been diagnosed with Heart Failure, contributing to a multi-disciplinary approach to the care of the cardiac patient.
Hours per week: 15.00

Main duties of the job
To plan and delivery evidence-based cardiac rehabilitation to those patients identified with coronary heart disease.
Provide a ward presence and identify appropriate patients for cardiac rehabilitation according to evidence-based guidelines
Ensure that each patient receives the necessary health education advice related to coronary heart disease and appropriate psychological support is provided for patients and/or carers throughout the cardiac rehabilitation process.
To participate in the Phase 3 cardiac rehabilitation programme in a gym setting which involves the delivery of cardiac related health education session and accompanying and supervising patients with their prescribed exercise prescription.
Demonstrate effective communication skills by liaising with other members of the cardiac rehab team and other health professionals, to ensure that patients and their families/carers are fully informed and have up to date and relevant information relating to their cardiac diagnosis and follow up cardiac rehabilitation.
To support and maintain patient safety when exercising in the leisure centre by undertaking individual clinical risk assessments and risk assess of the equipment and gym environment used for the delivery of the cardiac rehabilitation programme.
Maintain accurate patient records and ensures all relevant information is documented in the patients' medical and patient notes, including use of EMIS and Medisec.
