Advanced Clinical Specialist Physiotherapist - Community General Rehab

Job summary

East Cheshire NHS Trust has an exciting opportunity available for a suitably qualified Physiotherapist to become part of our established therapy team within the community. We welcome applications from Physiotherapists with a keen interest in working within an integrated community team, with proven clinical expert experience in bed based rehabilitation and general community rehabilitation.

This post is based at Knutsford Community District Hospital.

You will be required to be flexible at times and work across sites in East Cheshire to assist colleagues from other teams. It is therefore necessary to travel efficiently between sites and to rural areas.

Band 6 applicants with relevant experience are welcomed.

Hours per week: 30

Main duties of the job

Provide clinical expertise, making decisions about the best course of action for patients' care (both in bed-based rehabilitation and within the community setting). This will involve assessing patients and providing personalised rehabilitation plans, whilst working alongside multiple agencies, to determine the best management plan for patients.

To provide leadership and clinical supervision to junior staff members.

To challenge multi-disciplinary team discharge decisions to ensure all patients in cluster beds meet the Criteria to Reside.

Deliver programmes of supported patient rehabilitation, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that ensure patients can be maintained safely within their own homes, if possible.

To maintain and develop links with the wider multi-disciplinary team to support and improve community rehabilitation pathways across East Cheshire.
