Job summary
**This position is open to staff working at the Trust in the 'ECT Family' this includes currentEast Cheshire NHS Trust employees,bank staff, ISS staff and volunteers at the Trust.**
The Specialist Palliative Care Team is based in Hope House, Macclesfield District General Hospital. It is an integrated service covering the East Cheshire area and the General Hospital. The members of the team are MacMillan badged and work closely with other services and professionals in the locality.
Hours per week: 37.5
Main duties of the job
- Acting as key worker/advocate for patients with a palliative diagnosis. Striving to achieve quality personalised care.
- Working in collaboration with other clinical teams. Including Specialist Palliative Care and Lung Cancer Teams.
- working collaboratively with other teams both in the acute and community care setting.
- Presenting and discussing patients at SPC MDT.
- Being a source/resource of specialist palliative care skills and knowledge. Demonstrating advance knowledge and skills in this specialist area of care.
