Job summary

East Cheshire NHS Trust has an exciting opportunity available for a suitably qualified Assistant Practitioner to become part of our established therapy teams within the community. We welcome applications from Assistant Practitioners with a keen interest in working within an integrated community team, with proven experience in crisis response, in patient rehabilitation beds and general community rehabilitation.

We have a part time post available based at CHAW (Chelford, Handforth, Alderley Edge and Wilmslow) based at Handforth Clinic.

Hours: 20.25 hours per week

Main duties of the job

To work as part of a team providing a comprehensive, rehabilitation service for patients within a defined locality aligned to the care communities by the delivery of therapy independently or jointly with the therapists in individual, group and multidisciplinary settings. This may include supporting / delivering aspects of the pulmonary rehabilitation programme.

To contribute to operational delivery and service development

The community Therapy service is operational over 7 days, from 8am-8pm. You may occasionally be required to work a variety of shift patterns across the 7 day service, to meet service need.

  • BTEC National Diploma or equivalent in a relevant subject

Desirable

  • Assistant membership of a relevant professional body
  • Completion of a Foundation Degree in Health & Social Care
  • Successful completion or currently undertaking an ECDL

Knowledge

  • Knowledge / competency of a range of assessment tools & interventions relevant to the role
