Job summary
The Post holder will work collaboratively within the acute care setting as part of the Breast team, acting as a main point of contact for patients whoare undergoing active surveillance either within the Breast Cancer Stratified Pathway or within the Family history Service. In conjunction with the Clinical Nurse Specialists and the Advanced Nurse Practitioner you will provide administration support toassistin ensuring patients are managed appropriatelyon both pathways, collecting and collating informationand investigation results andmaintaintheappropriate databasein a timely manner.

Main duties of the job
To assistthe team to ensure allpatientsletters on the Stratified Pathway and Family History Pathway are completed and admin reviewed against guidance.

Triage incoming calls, usinggood communicationskills, basic clinical awareness andappropriate toolsand procedures, liaising asappropriate whennon routine and refer complex decisions to the Clinical Nurse Specialists, Advanced Nurse Practitioner or if needed Clinician for assessment and review.

Coordinate the necessary assessment, appointmentsor investigations tofast trackpeople back into the system if required.

Communicate and signpost toappropriate needsrelated information.

Makepre plannedoutboundtelephonedcalls topatienttoenable a proactive prevention approach.

Identifyindicators of need, or changesin need through telephone contact and respond appropriately.

Coordinate remote surveillance and monitoring through appointments for investigations
To provide administrative support for the Breast Cancer Now Health and Wellbeing Course,

Toassistthe CNS team with consent referrals for the Breast Cancer Now Health and Wellbeing courses
